I have done it on Tata Indicom network and have a Macbook Pro as well as a Macbook. Your SIM card must be pre-activated on windows or a CDMA phone.

1. Boot your Mac

5. Go to Location. Make a new location. Say Tata Indicom CDMA

6. Go to Show. Select Qualcomm CDMA Technologies
7. Enter Service Provider to identify connection.
8. Account name for Tata Indicom CDMA: internet
9. Password for Tata Indicom CDMA: internet
10.Telephone Number for Tata Indicom CDMA: #777

11. Click PPP options. Uncheck all but the ones that are checked in the snapshot below.

12. Go to Modem. Select au cdmaOne Bluetooth Adapter
13. Uncheck Enable error connection and compression in modem
14. Uncheck Wait for dial tone before dialing
15. Check modem status in Menu Bar.
